    From a Bedside Dream to Opening Day: Building My Hispanic Clinic

    Hey guys, my name is Alaina Hernandez. I'm a nurse practitioner and clinic owner of a Hispanic clinic. When I graduated nursing school, I quickly realized that I wanted to go back to nurse practitioner school. Through the first couple of years of nursing, I saw a true gap in Hispanic healthcare. I would see so many people coming in to the ER because they did not seek preventative care.

    So I started thinking, why is the Hispanic population in our area not seeking preventative care? And it just became very evident that there was not enough primary care accessibility within our community. So I decided, you know what, I'm going to go back, get my nurse practitioner degree and open my clinic. That truly was in my mind from the very beginning of starting nurse practitioner school. I really knew that I wanted to open my own clinic and I really knew that it had to be accessible to the Hispanic population.

    I decided to start The CLiNiCA Blueprint because I remember laying in bed and envisioning the clinic. I pretty much knew the location that I wanted to be in. I remember visualizing the front desk, who would work there. I had worked in the ER for some time and there we did our own labs, so I even envisioned my own little mini lab there. I knew what machines I was going to use. I pretty much had it all planned out. But I did not have the actual business plan. I didn't have the supervising physician. And I truly had no idea how to even run a business.

    I just remember it being just an idea, just a plan that just kept running through my head and wouldn't let me sleep. So at one point I finished my nurse practitioner degree and I went to work for another person in private practice. I quickly saw that it was doable. I wanted to do that. And honestly, I was commuting a little bit. It was difficult for the family. And I was like, you know what? It's been on my heart for so long to open my own clinic. I just want to jump in and do it.

    I kept hearing from people how hard it was. I remember when I first started talking about wanting to own my own clinic when I was still in nurse practitioner school, I would get pushback from a lot of people. Like, my goodness, it's so hard to own your own clinic. There's so many regulations. There's so much headache behind it. And while they're not lying, it's just that negativity really makes you feel like you can't do it. So that's why I decided to make The CLiNiCA Blueprint—because I wanted people to be able to see that yes, it's doable. You can have those dreams and you can make it happen.

    So if you're one of those people out there that are laying in bed at night envisioning your own clinic, just know that you can make it happen. It is a lot of work. It takes a lot of work on your behalf and you'll be working in the business for quite some time. But it truly is very rewarding. There's not a day that I regret starting my own clinic. It has allowed me to have opportunities with my family that I would not have if I was still working for someone else. I'm able to plan my schedule, my clinic schedule around my family schedule. I'm also able to close on days that the kids need to be out of school. So it's just truly a great thing for me and I do not regret it at all.

    When I think back to before I opened my clinic, I remember thinking about how do I start an LLC? How do I find a healthcare lawyer that can help me with some of the contracts? I remember just going through all the things. And now that I own a clinic and I've been through it, it definitely feels like I've overstressed myself way too much about the startup process. So I do know that it is stressful and you have the right to be stressed. It is a big responsibility. But I wanted people to know that hey, this is doable. And I'm actually going to give you a broad overview of everything you need to know about running a clinic as someone who owns a clinic.

    Another thing that I want The CLiNiCA Blueprint to be for people is a guide on what mistakes not to make. So I made quite a bit of mistakes when I got started. I bought some equipment that I really didn't need to buy. I probably didn't invest in mentorships where I should have invested in them.

    Some other things that I want The CLiNiCA Blueprint to be is a guide to help you decide what type of payment model you want within your clinic. Not all clinics can run on cash only. It's just not sustainable. And a lot of mentors out there will not teach you how to run an insurance-based practice. Ours is hybrid. So in this instance, I can teach you how to do both. But a lot of mentors stick to the cash is better—which obviously, getting paid for a service the same day of doing the service, that is much more favorable than billing insurance. But the reality is I want to help patients, even the ones that have insurance. So we are a hybrid practice.

    I had to navigate the struggles of learning how to credential, learning how to bill, learning how to navigate all of that. And it was a lot because there was not a lot of guidance there. So I want to be the person that can also help people start insurance practices or hybrid practices when that's what they want to do. Because I highly recognize that cash-based practices are amazing, but insurance-based practices are more scalable because more people have insurance than don't. And if people have insurance, they want to use it.

    Now I want to talk a little bit about what took longer than expected, and that is definitely the credentialing process. If you are starting a cash-based, only cash-based practice, you can really get from startup to running in a month or two. It is not that hard. Now, when you're working with insurances and you're credentialing, the credentialing process for some insurances takes about a month, but for others it can take anywhere from three to six months. I will say the longest insurance credentialing process for me was about three months. So don't let that discourage you, especially if you're going to be doing the credentialing process yourself. I honestly did most of the credentialing myself and it was pretty quick.

    Another thing that I want to talk about here briefly is how balancing patient care with responsibilities as an owner is very difficult in the beginning. At first I really didn't know anything about business, so I was taking courses on how to do my own books, how to stay financially stable. And so that responsibility was hard because I was seeing patients at the same time and I was juggling both. I didn't initially set aside CEO time, as they call it. So that's a very important thing to keep in mind as you're getting started with your clinic—making sure that you have CEO time where you're going to be reviewing your financial situation and paying attention to all of those things.

    The one thing that I would do differently if I was starting over from the beginning is really working with people that have expertise. My thought process was I don't want to spend the money because I think mentorships are pretty expensive. But since I've opened the clinic, I've actually done a mentorship and saw the benefits that I could have had from the beginning of the clinic. I think that I lost a lot of time and money not using a mentor. So if I would have just invested that money in the beginning, it would have made the clinic run smoother and honestly, I would have made less mistakes.

    Now I'm going to reflect on opening day. We did a ribbon-cutting ceremony a couple of days before we actually opened. And I just remember seeing everybody in the waiting room and just being overwhelmed with emotions because it was so exciting to see the support from the community, the support from family, and honestly just that feeling of being so proud of myself for what I was accomplishing.

    The first day of patient care was a good day, but it was stressful because you've got all new staff, you've got all new systems, and you're really trying to juggle care with working through workflow problems. And honestly, at the time I was the only one that knew how to correct any problems. So any lab problems, any medications, they didn't know where things were at. It all came down to me. If I was doing things differently again today, I would definitely have staff come in sooner and help me stock things so that they would know where stuff was. We actually saw fifteen patients on opening day, and to this day I'm like, how did that happen? Praise the Lord, we never saw zero patient days.

    Opening the clinic has been so amazing for me. It has opened so many doors professionally. I have made contacts with people that I would have never made contacts with. Serving the Hispanic community is truly the best thing I ever decided to do. For the most part, I have really good relationships with my patients. My patients know that this is a Hispanic practice. We're going to make sure that from the time you walk in the door, actually from the time you find us on Facebook or Google, from the time you go to our website, from the time you make an appointment to the time you come in the door, even when you go to the pharmacy to pick up your medications, everything is going to be accessible in Spanish.

    So I just want to let you know today, if you are considering opening your practice, whether it be cash only, hybrid model, insurance only, I can help you. Just don't get discouraged. You're going to hear so much negativity, but do not get discouraged. You can do this. It does take hard work, but honestly, it is so worth it. And the freedoms that you have from owning your own practice mean you really get to practice medicine the way you see fit. You don't have to listen to corporate rules. You don't have to work eight to five Monday through Friday and work your life away. You can work three days a week.
